ABSTRACT

Objective:To explore the social and physical anhedonia and its relationship to intrinsic motivation in patients with schizophrenia.Methods:One hundred and twenty-five stable schizophrenic patients from three psychiatric hospitals in Hefei, Wuhu and Beihai, and 101 healthy controls from same communities were recruited.All subjects completed Chinese version of revised social anhedonia scale(RSAS-C), Chinese version of revised physical anhedonia scale(RPAS-C) and intrinsic motivation inventory for schizophrenia research(IMI-SR), while positive and negative syndrome scale(PANSS) and Calgary depression scale for schizophrenia(CDSS) were used to assess the clinical symptoms of schizophrenic patients.All analyses were conducted by SPSS 26.0 software.The Mann-Whitney U test and covariance analysis were used for comparison between the groups, and Spearman correlation and multiple logistic regression analysis were used to explore the association between the anhedonia and intrinsic motivation in schizophrenics. Results:Compared with controls, the RSAS-C (10.00(6.00, 14.00) vs 11.00(8.00, 15.00), Z=-2.187, P<0.05) and RPAS-C (12.00(7.50, 20.00) vs 16.00(10.00, 23.00), Z=-3.026, P<0.01) scores in patients were higher, but the differerce between the groups disappeared after controlling age, sex and years of education.The IMI-SR perceived choice subscore (31.00(28.00, 39.00 vs 36.00(31.00, 42.00), Z=-3.172, P<0.01) were lower, while value/usefulness subscores (41.00(35.00, 45.00) vs 36.00(32.00, 42.00), Z=-3.387, P<0.01) were higher in patients than those in controls, and there was no significant difference between the total score and interest/enjoyment subscore(both P<0.05). In patents, Spearman correlation analysis showed that the RSAS-C and RPAS-C scores were significant negatively correlated with the IMI-SR total scores and interest/enjoyment subscore, perceived choice and value/usefulness( r=-0.193--0.364, all P<0.05), which still existed after controlling age, sex, years of education, course of disease, antipsychotic dose, and scores of PANSS and CDSS.Logistic regression analysis showed that the score of RSAS-C( B=-0.096, 95% CI=0.836-0.998, P=0.025) and perceived choice subscore( B=-0.110, 95% CI=0.823-0.974, P=0.010) had negative effects on the IMI-SR total score. Conclusion:There is a correlation between anhedonia and intrinsic motivation in patients with schizophrenia, the higher the social anhedonia, the lower the intrinsic motivation to participate in cognitive activities, suggesting that intervention for social anhedonia may have significance in improving the intrinsic motivation of patients with cognitive rehabilitation therapy.

ABSTRACT

Objective To establish a rational drug use model of PIVAS and promote the rational drug use in clinical practices by enhancing the quality of prescription review by pharmacists. Methods The PIVAS physician orders received from 2014 to 2021 were extracted through the hospital information management (HIS) system. The types of irrational physician orders were statistically analyzed, the improvements were made by the method of quality control circle (QCC). Results The model of PIVAS rational drug use formed a standardized process. The proficiency of physician order review was improved. From 2014 to 2021, the number and proportion of unreasonable physician orders in PIVAS decreased year by year. Every type of unreasonable physician orders was improved year by year. Conclusion The professional and technical levels of pharmacist for physician order review at our hospital were improved by the model of PIVAS rational drug use. The quality of pharmaceutical service was significantly improved which ensured the safety of patients' medication.

ABSTRACT

Malignant liver tumors have a high incidence and mortality rate. Therefore, it is of great significance to promptly learn about tumor advancement status through relevant examinations for patients' follow-up, diagnosis, and therapy as well as the improvement of the five-year survival rate. The primary lesions and intrahepatic metastases of malignant liver tumors have been better demonstrated in the clinical study with the use of various isotope-labeled fibroblast activating protein inhibitors because of their low uptake in liver tissues and high tumor/background ratio, which provides a new method for early diagnosis, precise staging, and radionuclide therapy. In light of this context, a review of the research progress of fibroblast-activating protein inhibitors for the diagnosis of liver malignant tumors is presented.

ABSTRACT

ObjectiveTo conduct phylogenetic analysis of internal transcribed spacer 2 (ITS2) and chloroplast gene segments including psbA-trnH, rbcL, and matK of Sophora japonica cv. jinhuai resource samples from different geographical sources, and to explore the genetic diversity of S. japonica cv. jinhuai. MethodPolymerase chain reaction (PCR) method was used to amplify the nucleic acid sequences of ITS2, psbA-trnH, rbcL, and matK of S. japonica cv. jinhuai. Neighbor joining (NJ) method was used to construct phylogenetic trees, and Kimura 2-Parameter (K2P) model was used to calculate the genetic distance of different samples. MEGA and BIOEDIT softwares were applied for mutiple alignment and analysis of ITS2, psbA-trnH, rbcL, and matK sequences of S. japonica cv. jinhuai. ResultThe lengths of ITS2 sequence were 278-279 bp. The lengths of psbA-trnH were 289 bp. The lengths of rbcL sequence were 673 bp. The lengths of matK sequences were 786-792 bp. There were 3 mutation points in ITS2 and psbA-trnH, no mutation point in rbcL, and 13 mutation points in matK. The samples of S. japonica cv. jinhuai were clustered into two groups based on the phylogenetic tree constructed by ITS2 sequences. The sample of seedling tree in Baibao was clustered into one group, while the other 25 samples were clustered into another group. For the psbA-trnH sequence, the success rate of PCR amplification of 28 samples of S. japonica cv. jinhuai was 100%. The 28 samples of S. japonica cv. jinhuai were clustered into three groups based on the clustering results of psbA-trnH sequence. The sample of seedling tree in Shaoshui was clustered into one group. The five samples of grafting tree and seedling tree in Miaotou, grafting trees in Jiantang, Wenqiao, and Daxu, and seeding tree in Xianshui were clustered into one group. The other 21 samples were clustered into another group. The 26 samples of S. japonica cv. jinhuai were clustered into two groups based on the phylogenetic tree constructed by matK sequences. The sample of seedling tree in Xianshui was clustered into one group, while the other 25 samples were clustered into another group. The clustering results of the rbcL sequence of S. japonica cv. jinhuai could not distinguish 28 resource samples. The phylogenetic tree constructed by the combined sequence of ITS2+psbA-trnH+rbcL+matK divided S. japonica cv. jinhuai resource samples into 4 groups. The 13 samples of seedling trees in Qiyang, Daoxian, Miaotou, Shaoshui, Shitang, Xianshui, Jiantang, and Xiangli, and grafting trees in Qiyang, Miaotou, Yongsui, Wenqiao, and Yangtang were clustered into one group. The sample of seedling tree in Wenqiao was clustered into one group. The sample of seedling tree in Daxu was clustered into one group. The remaining samples were clustered into another group. ConclusionPhylogenetic and mutation analysis provide the theoretic foundation to investigate the evolution of the resources of S. japonica cv. jinhuai, and evaluate their genuineness. The results of mutation points can be used to identify the related S. japonica cv. jinhuai resources. The findings of this study show that the combination of different gene sequences has an optimal effect on plant identification.

ABSTRACT

Objective:To evaluate the predictive value of pre-ablation stimulated thyroglobulin (Pre-sTg) and thyroglobulin reduction index (TRI) before 131I treatment for prognosis improvement in medium-low risk differentiated thyroid carcinoma (DTC) patients. Methods:One hundred and ninty-four patients with low-medium risk DTC who received 131I treatment at the Department of Nuclear Medicine of our hospital from July 2019 to June 2021 were included in to this prospective study. Multifactor Logistic regression analysis was performed to evaluate the influence on the prognosis of DTC patients. Results:Pre-sTg and TRI were the prognostic factors for radioiodine remnant ablation (RRA) ( P<0.05). When the cut off value for Pre-sTg was ≤8.64 ng/ml, the sensitivity, specificity, positive predictive value, negative predictive value and crude agreement for Pre-sTg were 71.43%, 62.96%, 83.33%, 62.96% and 69.07%. While the cut off value for TRI was >84.80%, the sensitivity, specificity, positive predictive value, negative predictive value and crude agreement for TRI were 54.29%, 70.37%, 81.72%, 28.24% and 58.25%. Conclusions:Pre-sTg and TRI are independent predictors of the efficacy of radioiodine remnant ablation (RRA), and can be used for early prediction of the efficacy of RRA treatment.

ABSTRACT

Objective:To construct and validate a nomogram model based on clinical factors and PET/CT metabolic parameters of 18F-FDG for predicting epidermal growth factor receptor (EGFR) mutations in lung adenocarcinoma. Methods:From January 2014 to January 2019, 114 patients (59 males, 55 females, age (60.0±10.8) years) with lung adenocarcinoma in the First Affiliated Hospital of Harbin Medical University were retrospectively enrolled. Clinical data (smoking status, tumor location, clinical stage and carcinoembryonic antigen (CEA) level), 18F-FDG PET/CT metabolic parameters (SUV max, metabolic tumor volume (MTV) and total lesion glycolysis (TLG)) and EGFR mutation status were analyzed. Patients were divided into training group (80 cases) and validation group (34 cases). In the training group, univariate analyses (independent-sample t test, Wilcoxon rank sum test, χ2 test or Fisher′s exact probability method) were used for categorical variables. Variables that showed significant differences between EGFR mutation group and wild type group were selected. Variance inflation factors (VIF) were calculated and the collinearity variables were deleted, and a nomogram model of optimal logistic model was constructed based on Akaike information criterion (AIC). The effect of the model was evaluated by the concordance index (C-index), sensitivity, specificity, accuracy, calibration and decision curve analysis (DCA) in the training group and the validation group. Results:Among 114 patients, 56 were with EGFR mutations and 58 were with EGFR wild type. In the training group, there were significant differences in gender (male/female: 14/26 vs 25/15; χ2=6.05, P=0.014), smoking status (with/without smoking history: 4/36 vs 22/18; χ2=18.46, P<0.001) and SUV max (5.72(3.90, 8.32) vs 8.09(4.56, 12.55); W=1 045.50, P=0.018) between EGFR mutation group and wild type group. However, there were no significant differences in other factors ( t=-0.54, χ2 values: 0.20 and 0.20, W values: 921.50 and 983.00, all P>0.05). The VIF of gender, smoking status and SUV max were all less than 10, and the nomogram model with three factors showed the minimum AIC (90.06). In the training group, C-index value of the model was 0.798 (95% CI: 0.699-0.897), with the sensitivity of 85.0%(34/40), the specificity of 70.0%(28/40) and the accuracy of 77.5%(62/80). In the validation group, C-index value was 0.854(95% CI: 0.725-0.984), with the sensitivity of 13/16, the specificity of 14/18, and the accuracy of 79.4%(27/34). The calibration curve and the goodness of fit test showed good calibration, and DCA showed that the model could benefit patients clinically within a large risk threshold range (training group: 0-0.59, validation group: 0-0.65). Conclusion:The nomogram model based on gender, smoking status and SUV max can be used to easily predict EGFR mutation status in lung adenocarcinoma.

ABSTRACT

Objective:Apt-A10-3.2 (aptamer of prostate specific membrane antigen (PSMA)) can be used as a specific ligand for early diagnosis and targeted treatment of prostate cancer. Mouse double minute 2 homolog (MDM2) is closely related to the malignancy of prostate cancer, and MDM2 small interfering RNA (siRNA) can silence MDM2 gene through RNA interference. To design a novel chimera of PSMA Apt-MDM2 siRNA and combine it with docetaxel (DTX) to explore a new diagnosis and treatment model combining targeted therapy of PSMA-positive prostate cancer with 99Tc m-chimera imaging monitoring. Methods:Apt-siRNA were obtained by covalent connection of PSMA Apt-A10-3.2 and MDM2 siRNA, which was combined with DTX to treat PSMA-positive prostate cancer cell lines (22RV1 and LNCaP). Cell lines were treated with Apt-siRNA alone or in combination with DTX. The levels of MDM2 and apoptosis-related proteins (B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X (Bax), poly ADP-ribose polymerase (PARP), caspase-3) were detected by Western blot, which were used to evaluate the therapeutic effect. Fifteen BALB/c mice bearing 22RV1 xenografts were treated with PBS, DTX+ Apt-siRNA (200 pmol) and DTX+ Apt-siRNA (400 pmol), respectively. Tumor volume and MDM2 level were observed, and 99Tc m-Apt-siRNA SPECT imaging was performed to obtain the tumor/muscle (T/M) ratio. One-way analysis of variance, Tukey′s test and linear regression analysis were used for data analysis. Results:The levels of MDM2 protein were significantly decreased by Apt-siRNA (0.25±0.02, F=183.40, P<0.001; 0.56±0.03, F=37.15, P<0.001) in 22RV1 and LNCaP cells. After the treatment of Apt-siRNA+ DTX, the levels of Bcl-2 were significantly decreased, and the levels of Bax, PARP and caspase-3 were significantly increased. MDM2 protein level (400 pmol: 0.59±0.12; F=49.99, P=0.023) and tumor volume (400 pmol: (0.22±0.07) cm 3;F=71.30, P=0.039) were significantly inhibited by Apt-siRNA+ DTX in mice bearing 22RV1 xenografts. As for 99Tc m-Apt-siRNA SPECT imaging in vivo, T/M ratio of treatment group was significantly decreased (400 pmol: 2.07±0.22; F=34.99, P=0.022), and there was a linear regression relationship between T/M ratio and the expression level of MDM2 ( R2=0.875, P<0.001). Conclusion:Apt-siRNA combined with DTX can effectively inhibit the progression of prostate cancer, and realize visual targeted diagnosis and treatment of PSMA-positive prostate cancer by coupling radionuclide technetium.

ABSTRACT

Case-oriented Workshop training mode is a professional training method which has been highly praised by anesthesiologists in recent years. It is of great significance to stimulate the participation of anesthesiologists and strengthen the application of the extended focused assessment with sonography for trauma (EFAST) in perioperative anesthesia management. The training of EFAST is divided into two stages: the first stage is about the relevant knowledge of EFAST and watching standardized operations on site, and the second stage is for each trainee to practice the operations under the guidance of the teacher until they can complete them independently. The EFAST adopts Workshop training mode, and the trainees can complete the target area scanning in about 90 seconds on average. It can maximize the training effectiveness and comprehensively improve the trainees' mastery of knowledge and skills in an all-round way. It is a training mode worthy of promotion in anesthesiology department.

ABSTRACT

Objective::To study the correlation between endogenous hormone content, related enzyme activity and embryogenesis during the stratification of Acanthopanax senticosus seeds, and to provide theoretical basis for application of endogenous hormones and related physiological indicators in regulating germination of A. senticosus seeds in production and scientific research. Method::Endophytic fungi as well as different concentrations of nitric oxide and hydrogen peroxide solution were used for soaking the seeds of A. senticosus, and then thermophilic stratification was conducted for the seeds. The content of endogenous hormones such as gibberellin (GA3), abscisic acid (ABA), indole acetic acid (IAA), indole butyric acid (IBA) and salicylic acid (SA) in seeds of A. senticosus were tested by high performance liquid chromatography (HPLC), and the activity change of enzymes in vivo such as catalase (CAT), superoxide dismutase (SOD), peroxidase (POD), and malondialdehyde (MDA) were tested. The correlation between the embryo rate and the hormones and their enzyme activities in the seeds of A. senticosus was studied by the grey correlation method. Result::The embryo rate was significantly positively correlated with IAA, GA3, and CAT, with correlation coefficient of 1.086, 0.935 and 1.067, respectively. There was a significant positive correlation between IAA, GA3, CAT, IBA, and SA, but with a significant negative correlation with ABA, MDA, and POD. The correlation degree was as follows: IAA>CAT>GA3>IBA>SA>SOD>ABA>POD>MDA. Conclusion::IAA, GA3 and CAT have significant promoting effects on embryo rate, and there is a significant correlation between hormone content and enzyme activity, which provides a basis for exploring the seed germination mechanism of A. senticosus.

ABSTRACT

Objective:To improve the quality standard of Shenwei Gubi tablets, and to explore the reasons for the great difference in the contents of quality control index components between batches of this product. Method:The fingerprint of this product was established by HPLC, the determination was performed on Diamonsil C18 column (4.6 mm×250 mm, 5 μm) with acetonitrile (A)-0.1% phosphoric acid solution (B) for gradient elution (0-5 min, 10%A; 5-15 min, 10%-12%A; 15-30 min, 12%-26%A; 30-43 min, 26%-31%A, 43-50 min, 31%-40%A, 50-70 min, 40%-55%A; 70-84 min, 55%-72.5%A) as the mobile phase at detection wavelength of 230 nm. The orthogonal partial least squares-discriminant analysis-variable importance in the projection (OPLS-DA-VIP) map was drawn with the common peak as the independent variable. The contribution of 26 common peaks to the fingerprint differences among different batches of this product was quantified. By searching for the chromatographic peaks with great differences, combined with relevant literature, the components related to the clinical indications of the product were screened out and their contents were determined by specificity experiment, and the quantitative indicators were finally selected. HPLC-doide array detector (DAD) was employed to determine the contents of the above preferred indexes with detection wavelengths of 236, 276, 230, 322 nm, other conditions were the same as HPLC fingerprint detection method. Result:A total of 26 common peaks were calibrated on the HPLC fingerprint of Shenwei Gubi tablets. The similarity between the fingerprint of each batch samples and the reference fingerprint was≥0.950. Loganic acid, gentiopicroside, paeoniflorin and osthole were optimized as the quantitative indicators of this product, their average contents were 161.02, 401.80, 255.54, 80.68 μg·g-1. Conclusion:The established fingerprint and multi-index quantitative analysis method are stable and reliable, and can be used for quality control of Shenwei Gubi tablets. Difference in contents of quality control components between batches of raw materials and the imperfect quality control method of intermediates in the production process are the main reasons for the great difference in the contents of quality control indicators between batches of this product.

OBJECTIVE@#To analyze the clinical features and genetic variants in a 13-month-old child with Bloom syndrome.@*METHODS@#Clinical data of the child was collected. Genetic variants were detected by high-throughput sequencing and Sanger sequencing.@*RESULTS@#The child was born at full term but was small for gestational age. His clinical features included loss of appetite, severe growth retardation, microcephaly, and small mandible. Genetic testing found that he had carried compound heterozygous c.1068+3A>C and c.1069-1G>C variants of the BLM gene, both of which were unreported previously.@*CONCLUSION@#Bloom syndrome is mainly characterized by severe growth retardation in infancy. The novel variants have expanded the variant spectrum of the BLM gene.

On the other hand, there are currently no antiviral drugs or vaccines available to treat some newly emerging virus infections. Therefore, antiviral drugs research has always been a hot research topic in the field of medicinal chemistry. Natural products are an important source of antiviral drugs. This article reviews the progress of antiviral natural products discovered in the past decade to provide potential lead compounds for drug development.

ABSTRACT

Objective To compare the predictive value of different ratio measurement in predicting the risk of malignant thyroid nodules and to determine the best value of ratio in diagnosing thyroid nodules.Methods The clinical data of 342 thyroid nodules diagnosed by ultrasonography and confirmed by histology in our hospital from January 2018 to August 2018 were analyzed.The ratio of nodules in different sections,including longitudinal plane ratio(A/T)and transverse plane ratio(A/T),was obtained through the maximum head-foot diameter(T),the maximum left-right diameter(T),and the anterior-posterior diameter(A)of transverse section measured by ultrasonography.The correlation of histological diagnosis of benign or malignant nodule with longitudinal ratio and transverse ratio were analyzed.Results The A/T and A/T of malignant nodules were 1.00(0.83,1.17)and 0.81(0.65,1.00),respectively,which were significantly higher than those of benign nodules [0.81(0.67,0.93)(=-6.567,=0.000)and 0.63(0.52,0.75)(=-7.239,=0.000)].The area under the ROC curve of A/T and A/T was 0.734 and 0.712,respectively,showing no significant difference(area difference:0.0210,standard error:0.0213,95% :-0.0207-0.0627,=0.987,=0.3235).The threshold values of A/T and A/T for predicting malignant nodules were 0.784 161 5 and 0.985 714 5,respectively.Conclusions Both A/T and A/T has similar diagnostic value in predicting the risk of malignant nodules.The best cutoff value of the above two ratios are 0.78 and 0.99 respectively.

ABSTRACT

Objective: To investigate the effect of exogenous nitric oxide (NO) on breaking the dormancy of Acanthopanax senticosus seeds and the changes in endogenous hormones and enzymes,and provide a basis for breaking the dormancy as well as artificial cultivation of A. senticosus seeds.Method: Different concentrations (1,5,10,20 mmol·L-1) of sodium nitroprusside (NO donor) were used to treat the A. senticosus seeds, and then thermophilic stratification was conducted. The content changes of endogenous hormones such as gibberellin (GA3), abscisic acid (ABA),indolo acetic acid (IAA),indolo butyric acid (IBA) and salicylic acid (SA) at different stratification time (0, 30, 50, 80, 100,130 d) were tested by high performance liquid chromatography (HPLC). The activity change of its in vivo enzymes[catalase (CAT), superoxide dismutase (SOD), peroxidase (POD), and malondialdehyde (MDA)] were tested by enzyme-labeled instrument.Result: In the seed germination process of A. senticosus,the contents of GA3,IAA,IBA,and SA were increased gradually,while the content of ABA was reduced gradually. The enzyme activities of POD and MDA were significantly reduced,and the enzyme activities of CAT and SOD were increased obviously. Exogenous NO could increase the seed germination rate and shorten the seed germination time. The effect of 20 mmol·L-1 sodium nitroprusside showed the most obvious effect and 10 mmol·L-1 SNP showed the weakest effect in promoting seed germination,showing an obvious "V" shape for changes.Conclusion: Sodium nitroprusside could promote the seed germination effect of A. senticosus, probably by increasing the content of hormones and enzyme in the stage of seed germination and improving the contents of endogenous NO during germination.

Objective To investigate the effect of alogliptin on albuminuria in patients with early type 2 diabetic kidney disease (DKD) and the related mechanism.Methods One hundred patients with early DKD admitted in our hospital from May 2016 to May 2017 were randomly divided into two groups with 50 cases in each group.Patients in the control group were given metformin and gliclazide,while those in study group were given metformin and alogliptin,the treatment lasted for 24 weeks.The changes of urinary albumin-to-creatinine ratio (UACR),stromal cell-derived factor-1α (SDF-1α) and the fasting plasma glucose (FPG),2-h postprandial plasma glucose (2 hPPG),glycosylated hemoglobin(HbA1c) were measured before and after the treatment in two groups.Results There were no significant differences in HbA1c [(8.17± 0.46)% vs.(8.29±0.48)%],UACR[(109±53) vs.(105±48)mg/g],SDF-1α [(1.21±0.3 9) vs.(1.17±0.35)μg/L] levels before treatment between two groups (t=0.343,0.464,0.075,all P>0.05).After treatment,the HbA1c levels were significantly decreased in both groups (t=2.293,2.302,all P=0.03) and there was no significant difference between two groups[(6.82±0.75)% vs.(6.93 ±0.79)%,t=0.295,P=0.77];the UACR levels were significantly reduced in both groups,but the level of study group was significantly lower than that of control group [(82±38) vs.(94±47) mg/g,t=3.320,P<0.01];the SDF-1α levels were significantly increased in both groups,but the level of study group was significantly higher than that of control group[(3.01 ±0.38) vs.(2.76±0.42)μg/L,t=5.474,P<0.01].There was no significant difference in the incidence of adverse reactions between the two groups [13% (6/46) vs.12% (6/48),x2=0.002,P>0.05].Conclusion Alogliptin can effectively control the blood glucose,reduce urine albumin excretion and protect renal function in patients with early type 2 diabetic nephropathy,which is associated with the increased SDF-1α levels.

The NeuroGam software can be used for analyzing functional brain images based on voxel analysis.It has been widely used to study on abnormal brain functional diseases,such as dementia,Parkinson disease,epilepsy,and Moyamoya disease.The software can be used for both clinical individual analysis and large sample research.It has no technical limitation and is simple to analyze.It is more suitable for clinical practice,and can be used as a powerful tool for clinicians to analyze kinds of abnormal brain diseases.However,large samples,multi-center and prospective clinical studies should be carried out in the future to verify its application value.

ABSTRACT

The training of residents majoring in ultrasound needs not only to pay attention to the teaching of speculative knowledge,but also to "look at pictures and talk",which is based on a large number of diagnostic ultrasound cases,training the residents to find,discover and discriminate the lesions from the ultrasonic images to finally form a sharp recognition and diagnosis ability.This study used the instant messaging tool WeChat to establish a virtual learning platform and implement a case based teaching process through this platform,which can avoid the limitations of time and space,and make the communication between teachers and students faster.According to the questionnaire survey,residents generally believed that the training mode can effectively improve learning efficiency and professional ability and hope to add this training mode to the basis of traditional teaching mode.The results of resident case analysis showed that the diagnostic accuracy of residents gradually increased during the training process.The model of WeChat platform combined with case teaching can provide new ideas and methods for standardized training of resident doctors in ultrasound specialty.

ABSTRACT

Objective: To study the quality of Acanthopanax senticosus seedlings,develop grading standards and optimize the best analytical method. Method: Observing and measuring the plant height,diameter,leaf area,root length,chlorophyll content and other main agronomic traits of A. senticosus seedlings from different habitats(Baoqing,Qitaihe,Dongfanghong,Yilan,Acheng,Raohe Linkou and Yabuli) in each year. K-cluster grading method,principal component evaluation factor K-cluster analysis method,standard deviation method for grading,three classification methods were evaluated with different levels of seedling survival rate as indicators. Result: The direct K-cluster analysis method was used to determine the quality of A. senticosus seedlings as the best method. The seedlings of A. senticosus were divided into 3 grades,among in the first level the seedling height is ≥ 13 cm,the stem diameter is ≥ 0.37 cm,the root length is ≥ 8 cm,the leaf area is ≥ 28 cm2,the chlorophyll content is ≤ 31,the main origin is Baoqing,Qitaihe area. In the second level the seedling height is 8-13 cm,the stem diameter is 0.30-0.37 cm,the root length is 6-8 cm,the leaf area is 13-28 cm2,the chlorophyll content is 31-32,and the main origin is Acheng,Dongfanghong,Raohe area. In the third level the seedling height is 5-8 cm,the stem diameter is 0.26-0.30 cm,the root length is 5-6 cm,the leaf area is 5-13 cm2,the chlorophyll content is 32-38,and the main origin is Yabuli,Yilan and Linkou. Conclusion: In this experiment,the quality grading standards of A. senticosus seedlings were preliminarily established,Baoqing and Qitaihe can be used as a high-quality production area for breeding seeds in the Heilongjiang province.which provided the basis for quality evaluation of A. senticosus seedling planting and artificial cultivation.

ABSTRACT

Objective: To study the fingerprints of Paeoniae Radix Rubra in different habitats, and determine the content of five chemical components (gallic acid, albiflorin, paeoniflorin, benzoic acid, and benzoyl paeoniflorin) and systematically cluster them. The relationship between origin and content was analyzed by grey correlation degree to provide reference for the quality evaluation of Paeoniae Radix Rubra. Methods: Fingerprints of Paeoniae Radix Rubra from 21 different producing areas were constructed by high performance liquid chromatography. The results were classified by principal component analysis and systematic cluster analysis. The gray correlation degree method was used to process the index components and their relative correlations were calculated. Results: HPLC fingerprints of Paeoniae Radix Rubra from 21 habitats were established, 11 common peaks were confirmed, and five of them (gallic acid, albiflorin, paeoniflorin, benzoic acid, and benzoyl paeoniflorin) were identified. The similarity of Paeonia lactiflora was greater than 0.9, and the similarity of Paeonia veitchii was less than 0.9. It was divided into two categories by principal component analysis combined with cluster analysis. The results of grey correlation analysis showed that the relative correlation (ri) was the largest in Gansu, followed by Ganzi in Sichuan. Conclusion: There is a big difference in the relative yield of Paeoniae Radix Rubra in different habitats. This experiment provides a scientific basis for the quality evaluation of Paeoniae Radix Rubra by fingerprint analysis, principal component analysis combined system cluster analysis and grey correlation analysis method.

ABSTRACT

OBJECTIVE@#To summarize and evaluate the value of applying the thyroid imaging reporting and data system (TI-RADS) released by American College of Radiology (ACR) in 2017 of the thyroid classification, and to propose an optimized classification method based on the result to facilitate more accurate and precise risk stratification of thyroid nodules.@*METHODS@#In the study, 342 thyroid nodules assessed by 2017 ACR TI-RADS were retrospectively analyzed. Each nodule had a score, and all the scores of nodules were compared with the pathological results. The proportion of malignant nodules in different scoring ranges was obtained. The diagnostic efficacy of all nodules, nodules above 1 cm and less than or equal to 1 cm was evaluated by ROC curve, respectively.@*RESULTS@#The AUC of all nodules, nodules above 1 cm and less than or equal to 1 cm were 0.907, 0.936 and 0.717, respectively. With the increase of the scores, the proportion of benign nodules decreased gradually, and the proportion of malignant nodules increased, especially nodules of 4-6 scores increased significantly. Based on the proportion of malignant nodules with 3 scores, the proportion of malignant nodules with 4, 5 and 6 scores increased 1.6, 3.8 and 5.3 times, respectively. The proportion of malignant nodules with 6-8 scores was 81%-84%, while the proportion of malignant nodules with 9 scores or more was 93%-94%. According to the distribution characteristics of malignant nodules, the classification of TI-RADS was adjusted. TI-RADS 4 was divided into TI-RADS 4a, TI-RADS 4b and TI-RADS 4c, corresponding to 4, 5 and 6-8 scores respectively, while the nodules with 9 scores or more were divided into TI-RADS 5.@*CONCLUSION@#2017 ACR TI-RADS has high diagnostic value for thyroid nodules above 1 cm, but it is not so effective for the nodules less than or equal to 1 cm. According to the proportion distribution of malignant nodules in different scoring ranges, appropriate adjustment of classification will be more accurate and precisely predict the malignant risk of nodules.
